DOTS method affects the patient’s attitude in taking medication regularity. One of the DOTS components is the direct supervision of short-term. To ensure the regularity of treatment required a PMO (Supervisory Swallowing Drugs). However, the study found that direct supervision by the PMO should not run This research is a descriptive study. There are 2 families as the samples . The variables were family support as PMO on the conversion treatment of pulmonary TB. The research data was analyzed by qualitative approach and indeep interview. Support family as PMO on the first patient was included in the good support, while the support for the family as PMO in the second patient was included in less support. The result of treatment of the first patient in the first two months experienced a conversion, while the results of treatment in two patients did not undergo conversion. In assisting family support, motivated and provided knowledge about TB to patient who was one of the factors that influenced the conversion in the second month of treatment in the patients with smear-positive pulmonary TB. Based on the above result it was necessary to change the role of the PMO which only remind patients to take take medications and monitoring patients in swallowing medicine but it gave Head Edecation to patient and other family could be applied so that the success of treatment could be achieved.
