ABSTRACT
Various studies showed that the examination of pulmonary tuberculosis by chest x-ray examination and clinical symptoms have not been satisfactory, sodespite the AFB sputum examination in some areas withoutadequate facilities or well-trained personnel in microscopic examination makes it less than ideal for the inspection efforts pulmonary TB screening, the diagnosis of tuberculosis (TB) still rely on it as the gold standard (gold standard). The objective of this study was to determine the validity of the diagnosis of pulmonary tuberculosis based on clinical symptoms and radiological examination in pulmonary Hospital Surabaya. This was a non-reactive (using secondary data) descriptive study with a cross-sectional design, involving 86 subjects to calculate the validity of the diagnosis of pulmonary TB either based on clinical symptoms and chest x-ray, with the gold standard sputum smear test results. The results showed that the sensitivity of chest x-ray and clinical symptoms include cough more than 3 weeks, weight loss, and night sweats, for the diagnosis of pulmonary tuberculosis at Surabaya Lung Hospital amounted to 86.40%, while the sensitivity of each of them were 25 % and 0.8%. Positive Predictive Value (PPV) of chest x-ray
examination and clinical symptoms amounted to 87.60% and 85% respectively, and their Negative Predictive Value (NPV) were23.1% and 0.9%, respectively. The conclusion is thatdespite not ideal, chest x-ray examination is more reliable for the diagnosis of pulmonary TB compared to TB diagnosis based on clinical symptomsonly. To avoid manyimproper TB diagnosis, health facilities without proper microscopic examination facility is expected to provide more adequate x-ray facilities and expert radiologists for the diagnosis of pulmonary tuberculosis suspects before it will be confirmed using sputum examination as the gold standard.
