ABSTRACT

The success of health services in primary health centers might not be reached due to medication errors which result in adverse events to the patients. This incident is related to the complexity of the interactions, the magnitude of dependencies among system components, the high workload, and the large number of patients (> 90%) given drug intervention. The approach to solve the issue is based on the description of the characteristics of High Reliability Organization (HRO). According to Lekka (2011), aspects and processes that characterize HRO consist of: mindful leadership, safety culture, and mindful safety of health workers. As the basic in ensuring medication safety, the influence of mindful leadership on the safety culture of pharmacy staff in primary health centers in Surabaya was examined in this study. This research is explanatory, it was conducted on 40 primary health center samples of the available 62 primary health centers in Surabaya. Sample size per region was determined proportionally and sample
selection in each region was done randomly. Data was obtained by distributing questionnaires to the heads of health centers and the pharmacy staff, and then analyzed descriptively and with inferential using PLS SEM. Descriptive analysis result showed that the mindful leadership and the safety culture variable have not met the 80% performance belonging to the Pareto Principle while PLS SEM analysis results showed that there is influence of mindful leadership on safety culture.
