Central Java, Indonesia was the highest of leptospirosis case in year 2012 which total number was 129 cases of 239 cases. Klaten district is the second highest leptospirosis cases in Central Java since year 2011 – third quarterly 2013. The environmental risk factors were the major transmission of leptospirosis. The aim of this study was to describe the environmental risk factor of leptospirosis in Klaten district. The method used the quantitative descriptive with cross sectional approach. The sample was leptospirosis patients with clinical symptoms and Ig M positive in year 2013 that lived in Klaten District which was 27 patients. Data was obtained from Health Department Klaten district in year 2013. Data presented using table and percentage. The result showed job as farmer was 29.62%, flood contact was 11.1%, rat contact was 18.5%, rat in the house was 100%, and ponded water contact was 11.1%. The rodent was a reservoir of leptospirosis. Infected urine rodent can exist in water and mud for several years. It was concluded that the majority of risk factors was job, rat in the house, and rat contact. While the flood contact, ponded water contact, and rodent contact were small risk factor. It was suggested that need to keep house clean from rodent, to repair the house reconstruction, and to maintain the rodent control.
